Job summary

Supporting Children and Young People (up to 25 years old) with Autism and/or Learning Disabilities.

We are seeking to recruit individuals with a passion for helping young people and their families to get the right support from Health, Social Care, Education and other services.

You will join an established team of Dynamic Support Keyworkers in Liverpool working across a wide range of settings to coordinate and provide challenge to the system, thereby ensuring that young people and their families can access the right support to enable them to achieve better outcomes and remain safe in the community.

Dynamic Support Keyworkers work with young people with Autism and/or Learning Disabilities who have multiple vulnerabilities and are at risk of / or experiencing escalating risks linked to

  • admission to mental health wards
  • family breakdown
  • sexual or criminal exploitation
  • self harm and suicidal ideation

Main duties of the job

Dynamic support key workers understand that the parents/carers are the experts in the child or young person’s care needs – they are there to support and empower families, they do this by.

Being a point of contact for the family and the involved professionals across the agencies/organisations

Having regular contact with the family; listen to, support and work in partnership with the family, sharing information and following up on plans agreed

Providing of information and advice, helping families to build their own confidence and be in control of the choices open to them

Should a child or young person be at risk of admission to a Mental Health Setting, the dynamic support key workers will work closely with the family and alongside other agencies involved to try and prevent this happening or will remain involved to try to make the hospital stay as short as possible and to facilitate a smooth discharge.

The dynamic support key workers have a significant role in supporting with the Care, Education and Treatment Review (CETR) process.
