The Adult Community service line is advertising for a full-time Interface worker on a permanent basis. The Interface team is a new and exciting team. We are looking for people that hold a current relevant clinical registration or equivalent relevant experience in health, social care or within the voluntary sector. The team will be expected to work in conjunction with Internal and external partners in supporting proactive discharge planning and timely discharge for patients known to our Community Mental Health Teams within acute admission wards. The team will work alongside the adult community service line which delivers care to adult mental health clients across the five boroughs of the Trust. The service line employs over 700 staff and we care for up to 12,000 patients a year.

Main duties of the job

You will be working closely to support discharge planning for all patients under the active caseload of the community team who are admitted to an acute mental health bed, including patients in out of area placements. The successful candidate will be expected to focus efforts on behalf of individual patients at heightened risk of delayed transfer of care to support their safe discharge and continued recovery at home.

Person Specification

Training & Qualification

Essential

  • Educated to first degree level
  • Current professional registration in relevant clinical subject/practice e.g. RMN, OT, Social Work, psychology or equivalent relevant experience in health, social care or voluntary sector
  • Evidence of post qualification training/education
  • Ongoing evidence of continuing personal or professional development

Desirable

  • Training in providing supervision, mentorship or equivalent, or willingness to undertake training
  • Project management qualification (PRINCE2 or MSP for example).
  • A commitment to completing full quality improvement development as outlined in the trust strategy

Experience

Essential

  • Demonstrable experience of liaising with community agencies
  • Demonstrable experience of working with people who have experienced serious ongoing mental health problems
  • Demonstrable experience of working within a multidisciplinary team
  • Demonstrable experience of implementing the principles and practice of the Recovery approach

Desirable

  • Experience of implementing changes in working practices
  • Experience of using quality improvement methodologies
  • Experience of implementing and monitoring standards of care, including clinical audit and governance

Knowledge & Skills

Essential

  • Excellent organisational skills including excellent use of IT
  • Excellent communication skills, both verbal (eg contributing to discharge planning meetings) and written (eg report writing)
  • The ability to influence others in clinical decision making towards discharge with a solution focused approach
  • Ability to respond positively and flexibly to changing demands and priorities
  • Ability to work under pressure, autonomously and achieve deadlines
  • Knowledge of safeguarding policies and procedures

Desirable

  • Knowledge of the south west London health and social care system
  • Ability to develop and maintain communication with people about difficult matters and/or difficult situations
  • Sound knowledge and skills in risk assessment
  • Familiarity with budget control and management
