We are looking for an enthusiastic Nursing Associate to join our busy Intensive Home Support Service.
You will be working with a diverse population and will be responsible for providing community care for patients in their own homes.
You will need good communication and interpersonal skills and the ability to work both independently and as part of a team. You will be expected to use your own initiative to deliver holistic care within a supportive team environment. You will need to be a car owner driver with a full, current driving licence.
The post holder will contribute to shifts cover 08:30 – 22:00- 7 days per week, 365 days per year.
Main duties of the job
The post holder will work within a Multidisciplinary Team providing direct patient care to patients in the community.
The post holder will join a well-established team of Advanced Clinical Practitioners, Therapy Teams, Mental Health Practitioner, Senior Community Practitioners, Community Practitioners and Support Staff. We also work with General Practitioners and Consultants from the Acute setting.
The post holder will be expected to work over 7 days contributing to shifts covering the hours of 08:30-22:00hrs.

Essential
- Nursing Associate: Level 5 Foundation Degree
- Associate Nurse on NMC register
- GCSE Level 2 English Language or equivalent GCSE
- GCSE Level 2 Maths or equivalent
Desirable
- To have knowledge of current research and evidence based practice applicable to the post’s specialist area
Experience
Essential
- Able to manage the care for a group of people and prioritise workload with minimal supervision.
- Understand the principles of safe and effective administration and optimisation of medicines in accordance with local and national policies
- Able to teach/ support less qualified carers/learners.
- Understanding of customer awareness
- Able to take on board new ideas, skills and knowledge.
