Job Summary
We are looking for a Band 4 Community Nurse Associate to work within the Out of Hours Countywide community nursing team supporting patients with clinicalhealth needs.
As a nursing associate, you will be working within the multi-disciplinary team and neighbourhood team. You will provide high standard of nursingcare and interventions within the patient’s own home, with support from the wider team. The patient will be at the centre of the care beingprovided, ensuring patient and carer participation in decision making.
We provide a service seven days a week, working closely with other community services and PCN’s to provide high standard care.
You will have the opportunity to develop yourself, with extensive learning and development opportunities.
Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. We are ideally looking for people who have:o Experience of community nursing, palliative care, care of patients with diabeteso Educated to Registered Nursing Associate on the NMC Registero Experience in falls prevention work or willing to gain skillo Experience of caring for and supporting adults and older peopleo Experience of assessing patients and formulating care plans independently and where appropriate, with support from RNs in the team.o The ability to demonstrate a commitment to high quality service provisiono The ability to work flexibly and as part of a team, as well as individuallyo The ability to travel independently using own vehicleo Good time management and the ability to manage individual workloado Enhanced communication and inter personal skills
