Job Summary
Service users with severe and enduring mental illness are known to have poorer health outcomes than the general population. For this reason, the North London Mental Health Partnership is providing support to service users around their physical health needs.
The post holder will work together with team members conducting supervised specialist assessments of service users with a special interest in their Physical health.
They will lead on ensuring that service users with enduring mental health issues are not discriminated by virtue of their mental illness in terms of their physical health.
The post holder will lead on supporting service users with blood tests, BMI, ECGs, BM, maintaining Lithium registers, and other physical health checks as required.
Main duties of the job
Have informed knowledge and understanding in the psychological, social, physical and spiritual needs of all service users.
Actively participate in the care and rehabilitation of service users as per the plan of care determined by registered practitioners.
Maintain clear, timely, comprehensive, and contemporaneous written clinical records on all patients in accordance with Trust policies.
Utilising IT skills and knowledge to access and input clinical information and data to the Trust Electronic System (RiO) and Local Authority IT system.
Undertake a range of duties which involve direct patient care, under supervision of the physical health lead clinician or other senior clinicians in the team.
Assist in the coordination of care delivery for a case load of service users in the community with mental health problems including those with severe and enduring mental illness under the supervision of a registered professionals, in close collaboration with the Directorate, psychiatric multidisciplinary team, service users and other community agencies.
