Job Summary

The Urgent Care Therapy Team is a dynamic Therapy team of Occupational Therapists and Physiotherapists that operates from 8am to 8pm seven days a week. The post involves joining a seven day roster that includes regular weekend work and provides opportunity for flexible working hours and enhanced payment rates.

The team covers A&E, Acute Medical Unit and the Frailty unit and works closely with the MDT, Social services and community colleagues to prevent admissions and facilitate timely discharges.

Successful candidates will require good communication skills, commitment to multi-disciplinary and cross agency working, a proactive approach to personal, professional and service development and well developed organisational skills.

Applicants should be HCPC registered.

Applicants whose first language is not English and who are required to provide a language test certificate as evidence of their proficiency must ensure that it is, or is equivalent to, IELTS level 7.0 with no element below 6.5

Main duties of the job

1. To provide expert physiotherapy within A&E, Acute Medical Unit andFrailty Unit.

2. To promote the role of therapy in prevention of unnecessary admissions tothe acute setting and facilitating early, safe discharges.

3. To work in conjunction with acute multidisciplinary teams, Therapy Assisted Discharge Service, Homefirst, community services, social and voluntary care to provide high level patient care.

4. To provide comprehensive geriatric holistic assessments in the Acute frailty Unit working closely with the MDT.

5. To complete comprehensive, holistic, rapid assessments and implement care plans to enable successful early discharge back into the community.

6. To plan, implement and evaluate integrated care pathways for patients seen by the Urgent Care Therapy team and provide expert clinical leadership in the planning and development of standards and protocols.

7. To be an active member of a team delivering 7 day service provision to patients.

8. To work with other Clinical Leads to manage a fair, comprehensive 7 dayroster.

Person Specification

Qualifications

Essential

  • Degree/Diploma in Physiotherapy/ Occupational Therapy
  • HCPC registration
  • Accredited practice educator

Desirable

  • Relevant post-graduate qualifications
  • Member of special interest group
  • Member of Professional Body

Knowledge

Essential

  • Previous experience at a senior level in an acute hospital setting
  • Evidence of personal and professional development
  • Evidence of having supervised students and qualified therapy staff
  • Experience of leading and influencing others
  • Experience in leading and contributing to audit
  • Experience in service improvement
  • Expert knowledge in common medical and surgical conditions and elderly care needs
  • Up to date knowledge of current developments in healthcare provision and acute hospital care
  • Understanding of services available in the community

Desirable

  • Experience of working in, or with, community care.
  • Experience of service management and leadership
  • Experience of a joint working approach

Skills
