Job summary

Are you a motivated and compassionate Nurse
Associate looking to make a meaningful impact on the physical health of people
living with severe mental health difficulties? We are seeking enthusiastic Band
4 Registered Nurse Associates to join our innovative SHAPE (Supporting
Health and Promoting Exercise)
service.

This is an exciting opportunity to be part of a
service dedicated to delivering goldstandard physical health assessments,
personalised interventions, and proactive health promotion. You will play a key
role in reducing health inequalities and improving longterm outcomes for our
patients:

Delivering highquality
physical health checks and tailored interventions.

Supporting patients
to engage in weekly wellbeing sessions.

Encouraging
participation in accessible activities and active lifestyleenhancing
programmes.

Providing expert
signposting and health education to empower patients in their recovery.

Working
collaboratively across Trust sites, community settings, and in patients homes.

Hours: MondayFriday, 9am5pm

Travel: Across Herefordshire & Worcestershire as part
of community delivery. Please note that this role requires regular travel with
equipment to multiple and rural locations often with infrequent or no public
transport.

Main duties of the job

Responsibility for arranging and
providing physical health checks for patients with a serious mental illness.

Reviewing and evaluating
physical health and wellbeing care plans identifying, organising and
signposting to other resources as appropriate.

Providing
assessments/interventions/activities within your scope of practice and level of
responsibility.

Completing relevant paper
and electronic documentation in line with the associate practitioner role and
local processes, in a timely and effective manner.

Provide information,
advice and signposting to promote good physical health and wellbeing e.g.
Smoking cessation, healthy diet, exercise, alcohol consumption, employment
support etc.

Encouraging patients to access suitable
follow up monitoring and interventions for any physical health concerns.

Escalating any physical health concerns
to appropriate clinical staff including GPs.

Supporting the delivery of a group health
and exercise programme for these patients alongside other members of the team.

Working flexibly to provide
the service delivery need across Herefordshire and Worcestershire.

Working with external
stakeholders, signposting to supportive services particularly in the VCSE
sector.

Person Specification

Experience

Essential

  • Experience of working in teams under appropriate supervision as part of a multi-disciplinary team.
  • Experience in prioritising own workload.
  • Experience in working without direct supervision.

Desirable

  • Previous experience as a Healthcare Support Worker involved in direct patient care in a care environment.
  • Experience of dealing with the public in person and on the phone.
  • Experience in carrying out assigned duties/assisting qualified staff after training.
  • Experience in supervising others.
  • Insight into how to evaluate own strengths and development needs, seeking advice where appropriate.
  • Experience of providing and receiving complex, sensitive information.

Qualifications

Essential

  • Foundation Degree Nursing Associate qualification.
  • NMC Nursing Associate registration.
  • GCSE English (grade A-C) or equivalent.
  • GCSE Maths (grade A-C) or equivalent.
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.

Desirable

  • IT Qualifications.
  • CAVA or Assessors qualifications.

Knowledge

Essential

  • Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development.
  • Understands and acts in line with NMC professional standards for practice contained within The Code.
  • Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice.
  • Understands requirements for NMC professional Revalidation.
  • Understanding of the importance of following procedures and treatment plans.

  • Ability to develop effective and appropriate relationships with people, their families, carers and colleagues.
  • Ability to support, supervise, assess and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting.
