Rehabilitation Co-ordinator Single Point of Contact

Job summary

The Rehabilitation Co-ordinator acts as the Single Point of Contact(SPOC) for referrals into the Cheshire and Merseyside RehabilitationNetwork, providing expert clinical advice and coordination for patientswith complex rehabilitation needs. Key responsibilities includereviewing and triaging referrals, undertaking comprehensiveassessments, advising on the most appropriate rehabilitation pathway,and coordinating access to specialist rehabilitation services across theNetwork. The post holder works closely with acute hospital teams,rehabilitation units, community services, patients, families, and carersto facilitate timely transfers and ensure seamless transitionsthroughout the rehabilitation journey. The role involves supportingcomplex discharge planning, monitoring patient progress throughrehabilitation pathways, maintaining referral and activity databases,and ensuring key performance indicators relating to access, waitingtimes, and patient flow are achieved. The post holder also providesspecialist rehabilitation advice to healthcare professionals, contributesto service development and quality improvement initiatives, andsupports the Network’s aim of delivering equitable access to highquality rehabilitation services across Cheshire and Merseyside.

Main duties of the job

The Rehabilitation Co-ordinator will act as the Single Point of Contact (for referrals , supporting patients with complex rehabilitation needs toaccess the most appropriate service.Key responsibilities include reviewing and triaging referrals, undertaking assessments, providing specialist rehabilitation advice, and coordinatingaccess to rehabilitation services across the Network.Working closely with acute hospitals, rehabilitation units, community services, social care teams, patients and carers, the post holder will facilitatetimely transfers, support discharge planning, and reduce delays in rehabilitation pathways. They will monitor patients awaiting transfer, identifybarriers to progression, and escalate concerns as required.The role includes participation in multidisciplinary meetings, complex case discussions, stakeholder engagement, and providing advice onrehabilitation pathways.The successful candidate will contribute to service development, quality improvement, audit activity, and delivery of regional rehabilitationpriorities. They will maintain accurate records, monitor patient pathways, support performance reporting, and contribute to objectives relating topatient flow, waiting times, access, and outcomes.Working autonomously, the post holder will use advanced clinical reasoning and professional judgement to support equitable access torehabilitation services and improve patient outcomes. Regular travel across Cheshire and Merseyside will be required.

Qualifications

Essential

• Nursing or AHP degree or equivalent relevant professional qualification
• NMC or HCPC Registration

Experience

Essential

• Demonstrable post graduate experience in a rehabilitation setting
• Previous experience as an nurse or allied health professional with extensive experience of treating patients with neurological and/or multiple conditions
• Clinically relevant experience in neurology or trauma specialities within a relevant rehabilitation setting
• Ability to demonstrate effective team working within a multi-agency/network framework

• Managing and interpreting data
• Participation in change management

Knowledge

Essential

• Understanding of national and local policies and guidance
• Demonstrates an understanding of the rehabilitation ethos and has a high level of ability that will inform and enhance optimum recovery and quality of life
• Understanding of the Mental Health Act 1983 and Mental Capacity Act

Desirable

• Understanding of clinical audit and research practices relevant to rehabilitation.
• Demonstrable knowledge and experience of a range of rehabilitation functional outcomes measures and implications for long term recovery of life skills
