Main duties of the job

Providing patient care in their own homes and in hospitals.

To work within a delegated caseload with the focus of facilitating behavioral change to promote and optimize self-care.

To communicate information to patients and carers regarding their health needs and associated treatment plans.

To report and take appropriate action regarding any change in patient condition or circumstance and communicate verbal and written information to all relevant integrated care team members.

To contribute to and support the maintaining of service standards and performance through audit and bench-marking, and to encourage colleagues and team members to do also.

To participate, as an individual and as team member, in continuous learning and reflective practice to maintain own knowledge and skills, share good practice with and provide instruction to colleagues to continually improve services.

To continue to promote patient centered care and utilize the shared decision making tools.

To actively contribute to protecting people at risk by assessing and monitoring the care environment and individuals and by following and acting on and within Safeguarding policies and procedures.

Person Specification

QUALIFICATIONS / TRAINING

Essential

• GCSE Maths and English Grade 9-2 (A* – E) or equivalent
• Care qualification to NVQ 3 or equivalent experience
• Must be willing to participate in any relevant training/qualification identified to develop skills required to carry out duties e.g., Foundation degree, Higher National Diploma

Desirable

• Have completed training or study in areas relevant to the role applied for or be willing to undertake
• Evidence of previous learning related to clinical area to diploma level
• Evidence of continuing professional development.

KNOWLEDGE, EXPERIENCE & EXPERTISE

Essential

• Experience of working in health or social care settings without direct supervision
• Experience of relevant clinical area e.g., older people, pharmacy, Community.
• Experience in participating in clinical supervision.
• Experience of organising and prioritising own workload.
• Relevant experience of as part of a team.
• IT awareness and ability to use SystmOne.
• Relevant experience of working with patients and families from a range of cultures and backgrounds.
• Relevant knowledge related to best practice guidance for care of patients and their families.
• Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice.
• Understanding of the importance of following procedures and treatment plans

Desirable

• Skills in research and audit methodology or willing to undertake
• Experience of facilitating clinical supervision.
• Experience of participation in clinical audit.
• Experience of working without direct supervision.

COMMUNICATION AND RELATIONSHIPS (INCLUDING MANAGEMENT RESPONSIBILITIES)

Essential

• Experience of working in teams under appropriate supervision as part of a multidisciplinary team.
• Excellent interpersonal and communication skills. Ability to communicate effectively (written, verbal, non- verbal communication) with patients/relatives, carers and other members of the multi-disciplinary team.
• Ability to develop effective and appropriate relationships with people, their families, carers and colleagues.
• Ability to organise and prioritise own delegated caseload.
• Ability to deal with non-routine and unpredictable nature of workload and individual patient contact
• Insight into how to evaluate own strengths and development needs, seeking advice when needed.
• Experience of providing and receiving complex, sensitive information

Desirable

• Knowledge of clinical governance and accountability
• Knowledge of national and local influences on the provision of health and social care
• Understanding of professional roles relevant to clinical area e.g., nurse, therapists, pharmacists
