Job summary
This is a part time post working 30 hours per week.
We are have an
exciting opportunity for a Band 6 Senior Community Nurse who has a passion for admission
prevention to join a core Team of ACPs to deliver a new and developing countywide
Hospital at Home (H@H) service in Worcestershire.
The post is based
in the Single Point of Access (SPA) Worcester.
This is an
exciting opportunity to be part of the delivery of a new service and to play a
significant part in shaping and developing this service. There will be the
opportunity to partake in audit and service development as well as the clinical
duties of the post. There will also be the opportunity to work collaboratively
with stakeholders in particular with Acute Trust colleagues.
The
service will be countywide across Worcestershire so the ability travel across
the county will be expected. The service will be operational 8am-8pm seven days
a week including Bank Holidays so the post holder will be expected to work
shift patterns to enable cover of the operational hours.
Main duties of the job
There
will be the requirement to support the assessment and management of acutely
unwell patients in the Community who otherwise might have been in an Acute
Hospital setting with the support of a dedicated medical model for the Hospital
@ Home.
The
ability to work independently is essential as the post holders will need to
offer urgent care, treat minor illness, give patient advice and offer chronic
disease management. You will have the ability to prioritise and triage the
needs of patients, making any referrals for further investigations where
appropriate in line with local and practice guidelines, whilst ensuring your
clinical practice is safe and effective and escalating to clinical leadership
as required.

- Registered Nurse (RN1)
- Graduate Nursing Qualification at Academic Level 6 or working towards.
- Preceptorship training

- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Experience
Essential
- Significant post registration experience in a clinical setting able to demonstrate transferability of experience to community setting
- Experience of management of caseloads or wards and organisation of staff
- Clinical audit/service improvement and development work
- Experience of active participation in interdisciplinary team working
Desirable
- Previous community nursing experience
- Transcribing medication training
Knowledge

- High level of clinical knowledge and skill
- Understanding of personal contribution to the achievement of organisational objectives within the business plan
