Job summary

The States of Guernsey are looking for a Senior Mental Health Practitioner to join the Older Adult Community Mental Health Team.

The Older Adult Community Mental Health Team which incorporates the memory service which is based on the Island of Guernsey is looking for an experienced Band 6 Senior mental health practitioner. As a band 6 Mental Health Practitioner you will be part of a service that will provide specialist input to service users and their carers who may be affected by a mental health diagnosis. You will be part of a service that provides evidence based, first class healthcare putting service users needs at the centre of the service.

You will be expected to have a level of knowledge and expertise acquired through a proven in-depth experience of older adult community mental health needs, contribute to and provide assessment and post diagnosis support of patientes within the memory service and dementia care, underpinned by extensive post registration to develop skills, knowledge and values.

Able to work autonomously on completing assessments, formulating person centred care plans and risk assessments for those accessing the service. Knowledge and being part of Safeguarding processes, formerly assessing capacity and holding a valid driving licence essential.

Person Specification

Qualifications

Essential

  • A Qualified Registered Mental Health Nurse RMN
  • Post Registration courses at degree or diploma level
  • Active and working on the NMC register
  • Practice Supervisor or Mentor
