Job Summary

There is an exciting opportunity for a part time Social Prescriber to work within the Mental Health Access Team in the Central Cheshire locality.

Social Prescribing aims to reduce reliance on NHS services by working within clinical teams linking service users with established activities within their communities, to improve health and wellbeing and improve self management.

The role involves working within the clinical team undertaking short term work with clients to understand current needs, completing the relevant resource groundwork in order to provide the appropriate signposting and networking into external resources and third sector organisations.

You will need to have excellent communication, problem solving and organisational skills, and the ability to work within a multi disciplinary team.

Social prescribing is an alternative pathway where instead of psychological interventions the clients may benefit from social engagement to positively impact their mental wellbeing.

To apply for this post you will need to have a sound mental health background with previous experience in health or social care and a good working knowledge of third sector services.

Main duties of the job

Working closely with organisations to engage clients with those services, appropriate for their needs.

Giving out advice to colleagues concerning available resources that they themselves can signpost to.

Completing the necessary groundwork involving research and contact with the services available to tailor needs.

Qualifications

Essential

  • NVQ Level 2 or 3 in health or social care

Knowledge & Expertise

Essential

  • Knowledge of mild to moderate mental health problems and their presentation
  • Evidence of further professional development

Desirable

  • Personal development within current role

Experience

Essential

  • Experience of working in a multi-disciplinary team
  • Experience of working within a Primary Care Mental Health Team and Social Care team

Desirable

  • Community mental health team experience
