Job summary

We are looking for a thoughtful, proactive and enthusiastic individual to join the friendly Stroke team. The role would suit an experienced band 5 or existing band 6 Physiotherapist with demonstrable Stroke / Neurology, community experience.

As a community Physiotherapist you will be working with the Stroke Team, assessing and treating patients providing a high standard of care and physiotherapy services to patients with diverse and sometimes complex conditions in their usual place of residence. The service opperates across the Scarborough and Ryedale Locality providing treatment and care in patients own homes and places of residence. It is essential to have a car and driving licence for the UK to carry out this role.

You will be responsible for providing expert stroke physiotherapy assessment, treatment and advice, caseload management, service development as well as line management responsibilities, working closely with colleagues in this multi-disciplinary team and supported by senior Physiotherapists.

Main duties of the job

As a Community Physiotherapist, you will have access to supportive supervision and peer review from a well-established team of Physiotherapists. Within this role you will:

  • Perform comprehensive physiotherapy assessment of patients with diverse presentations, using clinical reasoning skills and manual assessment
  • Formulate and deliver specialist individual physiotherapy treatment programmes based on a sound knowledge of evidence-based practice
  • Formulate accurate prognoses and recommend best course of intervention
  • Evaluate patient progress, reassess and alter treatment programmes if required
  • To provide line management, supervision, support, guidance and training to junior staff and students

Person Specification

Qualifications & Knowledge

Essential

  • Health and Care Professionals Council (HCPC) registration
  • Specialist understanding of relevant clinical practice, standards and protocols within identified clinical area.

Desirable

  • Evidence of relevant CPD – could include completion of short courses or post-graduate study.
  • Develop specialist understanding of relevant clinical practice, standards and protocols within identified clinical area

Experience

Essential

  • Experience of Community Physiotherapy in prior NHS employment or student placement.
  • Experience of complex case load management, within specialist area.

Desirable

  • Experience or working in Community based physiotherapy
  • Experience of using SystmOne

Skills & Competencies

Essential

  • Can commute effectively to facilitate community based visits including transportation of equipment required for clinical interventions.
  • IT skills to facilitate use of electronic patient records & other electronic systems
  • Effective inter-personal skills and experience in supervising and mentoring

Desirable

  • Ability to identify and set up home based assistive equipment according to individual patient need
  • Ability to carry out basic observations and calculate NEWS2 score
