We have an exciting opportunity to work as part of the developing Diabetes Intermediate Service (Tier 3) – providing a robust link between primary and secondary care. The post holder will provide evidence basedcare, education,supportand advice to patients with all types of diabetes, and their families.Additionallythe post holder will be an integral part of the wider multi-disciplinary team, acting as a resource and providing education and support to other health care professionals involved in the management of patients with diabetes.

Main duties of the job

To take the lead role in diabetic foot care within Community Diabetes service working as part of a multidisciplinary team.

To continue to strengthen patient pathways between Acute, Tier 3 and Community care.

To lead and implement a regular review of outcome measures across the service to ensure the service is effective.

Provide an exemplary standard of care whichincluded highlyspecialist diagnostic assessment,treatmentand advice, including advanced wound management ofhigh riskgroups including vascular complications, tomaintainpatient’s mobility, independence and prevent disability.

To lead on clinical local and national audits, service pilots/projectsand other research activities to support the development ofevidence basedpractice.

To provide clinical support to the podiatry team in the community integrated diabetes service (Tier 3).

To act as a source ofexpertiseand undertake training of podiatry students, support colleaguesmaintainand develop their clinical skills in diabetes and woundcare, andeducate other health care professionals on foot care.

Qualifications

Essential

  • HCPC Registered Podiatrist (DPodM or BSc)
  • Local Anaesthetic Certificate

Desirable

  • Specialist clinical experience
  • Involvement in raining and teaching IR(ME)R

Experience

Essential

  • NHS experience. Experience in the provision of high risk treatments and diabetic assessments in an acute environmen
  • Broad spectrum of clinical experience Working independently as a team member
  • Able to demonstrate in-depth clinical knowledge and practical skills
  • Awareness of current NHS trends and policies, including clinical governance and NSF’s

  • Experience in clinical audit
