Job Summary

Hywel Dda University Health Board presents TWO exciting opportunities for motivated and experienced Physiotherapists to join our NHS & Public Health Award Winning – “Home First Team” (Intermediate Care Services) in Carmarthenshire.

We are an innovative, comprehensive team of Physiotherapists, Occupational Therapists, Therapy Assistant Practitioners, Dietitians, Nurses, GP’s, Advanced Paramedic Practitioners, Advanced Nurse Practitioners , Physician Associates, Social Workers, Senior Reablement Workers, and Third Sector Agents striving to deliver the right support, at the right time in the right place as close to home as possible.

The role will form part of the “Home First” Multidisciplinary Team (MDT), all of whom work together to provide early intervention, period of rehabilitation and assessment in our bed based and home-based community settings to immediately mitigate loss in function, risk of deconditioning, nutritional deficit, deterioration in their mental health to increase people’s time spent at home rather than hospital.

Main duties of the job

Our approach is to deliver patient care in a holistic, person-centred manner, supporting patients, when possible, to stay/return in a safe manner to their own homes where they may continue to be a valued member of their own community.

As a versatile postgraduate, you will need to have a strong clinical grounding in older people rehabilitation, expertise in frailty syndrome, neurological conditions, cardiorespiratory conditions, MSK problems and a good understanding of current issues affecting intermediate care and community services.

Successful applicants will require an excellent ability to work autonomously, managing and prioritising a diverse and unpredictable caseload, with the ability to adapt to a rapidly changing and pressurised environment alongside members of the MDT. Post holders will be expected to support & supervise junior Physiotherapy staff, Physiotherapy Technicians, Rehabilitation Assistants/Frailty Support Workers, and students.

Qualification and Knowledge

Essential

  • Professional registration as recognised by HPCP
  • Post graduate evidence of specialist education
  • CPD evidence
  • IT skills

Desirable

  • Post-graduate qualification up to post grad diploma level in relevant specialty
  • Clinical Education Training Course.
  • Member of C.S.P
  • Active participation of specialist interest groups

Experience

Essential

  • Post-graduate experience in relevant clinical specialty
  • Completion of junior competency framework
  • Practical experience to a level that would enable participation on the respiratory on call rota
  • Therapeutic handling and risk assessments
  • Managing a diverse caseload using a clinical reasoning process
  • Inter-professional and team working
  • Direct involvement with carers and families
  • Evidence of supporting service compliance with clinical governance requirements
  • Experience of staff supervision

Desirable

  • Experience of service development
  • Experience of NHS working
  • Evidence of student supervision
  • Negotiation of MDT working practices

  • Evidence of teaching / In-Service Training.
