Job Summary
An exciting and unique opportunity has arisen to be a part of the Mental Health Intensive Support Service based in Chester.
This service provides specialist assessment, treatment, interventions & support, to help people recover from complex mental health problems & to regain the skills & confidence to live successfully in the community.
Main duties of the job
As Support Time and Recovery Worker you will be expected to support the MDT in providing intensive rehabilitation support to a small cohort of patients, providing high quality levels of care in order to assist people to live more independent lives.
You will be working alongside the Primary Workers to ensure effective and needs-led patient care with a strong focus on dialogical practice, recovery, identifying relapse indicators, skill development and promoting independence. You will also be responsible for supporting engagement in the agreed care plan and assist in encouraging inclusion in local community services, to promote daily living skills.
As a service we value personal development,therefore we are looking for candidates with a willingness to develop, particularly in skills around dialogical practice/Open Dialogue.
Alongside the MDT you will take an active role in training of care staff/third party organisations supporting the patient, such as Supported Living or Care Agency staff.
MHIST are proactive in pre-empting crisis and preventing admission into hospital, and therefore you must be able to demonstrate excellent skills in risk management, proactive care planning and communication skills across a range of scenarios.
Candidates are welcome to apply from a range of health or social work backgrounds, or similar experience, however experience in working with complex patients is encouraged.
