Job Summary

The States of Guernsey are looking for a Support Worker to join the Community Living Team.

This role supports adults with profound and multiple learning disabilities within a 24hour Community Living service.

Support is delivered through outreach and in reach to individuals living in their own homes, extra care housing. Staff use the Active Support Model focusing on doing things with people, not for them promoting choice, independence, and participation at every opportunity to help service users participate in daily living tasks such as personal care medication management, cooking, cleaning, shopping, managing finances, and accessing the community.

Band 4 staff are required to work independently and alongside colleagues under the guidance of senior staff to provide high quality physical and emotional care, and they act as key workers in line with service values and quality standards.

The role also involves lone working, particularly during outreach visits or when supporting individuals in their own homes, requiring staff to work safely, independently, and confidently while following organisational procedures also providing 1:1 supervision to Band 3 Support Workers.

Additional duties may be assigned by managers. A driving licence is required, and staff must be willing to drive service vehicles.

Person Specification

Qualifications

Essential

  • Any of the below:
  • Band 3 competency pathway
  • Level 3 VQ Diploma in Health and Social Care
  • Level 3 BTEC Advanced Certificate in Positive Behavioural Support

Desirable

  • Approved programme of development at Diploma Level (Level 5 HE) or equivalent e.g. Level 4 BTEC Professional Certificate in Positive Behavioural Support.
  • Nursing Associate

Experience

Essential

  • Ability to demonstrate an understanding of HSCs CARE Values
  • Self-motivated and able to use initiative
  • Leadership / Shift leadership experience
  • Good level of verbal and written communication and record keeping skills.
  • To be friendly, approachable, patient and empathetic. Ability to organise self; be reliable, flexible, adaptable and able to work under
  • pressure.
  • Ability to lead, direct and support staff.
  • Experience of Person-Centred Planning
  • Good IT skills.
  • Working knowledge of Makaton or Signalong
  • Medication management and administration.
  • Be an advocate for service user involvement and social inclusion.
  • Ability to recognise areas of concern, problem solve and escalate as required.
  • Driving licence
