Job Summary

The States of Guernsey are looking for a Team Leader to join the Social Care Team within the Community Health and Social Care Service.The post holder will work closely with the Senior Team Leaders and Staff Nurses assisting in providing support and health and social care interventions to service users living in the community and will assist in maintaining continuity, consistency and equitable provision of care to service users in the community while supporting and guiding the Community Carers to ensure best practice is achieved.

 

Main duties of the job

The States of Guernsey is seeking to recruit a Team Leader/Support Worker to join the Social Care Team in Community Health and Social Care Services.

The post holder will work closely with the Senior Team Leaders and Staff Nurses assisting in providing support and health and social care interventions to service users living in the community.

The post holder will assist in maintaining continuity, consistency and equitable provision of care to service users in the community while supporting and guiding the Community Carers to ensure best practice is achieved.

Qualifications

Essential

  • Band 3 competency pathway, or
  • Level 3 VQ Diploma in Health and Social Care
