An exciting opportunity has arisen to expand our AHP team to meet the challenges of delivering the new model for community specialist palliative care-Compassionate Care for All.

These two roles are new to the hospice and community with one based with our Ealing team and the other with our Hounslow team.

We are looking for someone who wants to make a difference in caring for our palliative care patients and those important to them.

 

Main duties of the job

Working with patients both within the hospice and in the community to provide a high standard of care whilst ensuring that patient independence is maintained for as long as possible.

In this role you will:

Assist the physiotherapy and occupational therapy team in managing a caseload of palliative and end of life patients

Assist with planning and undertaking individual therapeutic assessments of patients and use findings to plan, provide and modify the patients’ ongoing programme

Continually monitor patients’ response to these programmes, evaluating outcomes and adapting input as appropriate, liaising with therapy staff and other members of the multidisciplinary team

Assess and discharge less complex patients with caseload independently

Understand and deliver on the principles of sensitive communication in a patient population where prolonged and sustained rehabilitation and may not be possible

Be accountable for the treatment delivery you provide, supported by your line manager

Person Specification

Education and qualifications

Essential

  • Working within a variety of therapy or related services
  • Experience working as part of a multidisciplinary team in healthcare settings
  • To have a working knowledge of a wide range of palliative conditions
  • To have knowledge of how these conditions will influence the rehabilitation of the patients

Desirable

  • Trusted Assessor qualification
  • Experience working in physiotherapy/Occupational Therapy health care settings
  • To have knowledge of role of occupational therapy and physiotherapy

Knowledge and experience

Essential

  • Evidence of continued professional development within the field of therapy and specifically within the area of work underpinned by tailored in-service training programmes to develop competency with relevant patient group
  • NVQ level III or Part I City and Guilds or experience to an equivalent level

Skills, abilities and attributes

Essential

  • Ability to devise simple rehabilitation programmes based on assessment findings
  • To possess technical skills including the recording of patient outcome measures on a spreadsheet and maintaining a patient database
  • Ability to present information, written and oral, in a clear and logical manner
  • Good communication and negotiating skills
  • Ability to carry out moderate to intense physical effort throughout the working day
  • Ability to work sensitively with emotional patients and carers
  • Will be required to operate hoists and other equipment, e.g. standing frames, after tuition
  • Valid driving licence and access to own vehicle

Desirable

  • Computer skills including Excel, data analysis skills
